Create Secure Online Form

Need the attached form turned into a secure online form that will generate an email to me directly when submitted and store data in a database for future reference. Need this done immediately - like TONIGHT!

Current hosting includes:

RedHat Linux

Apache with PHP4 and Perl5 support

qmail

Form should look EXACTLY like the attachment - very professional, very polished.

HIGH EXPECTATIONS for the visual as well as the functionality.

Want the process to be through secure server and not sure how that works with current hosting company which is [url removed, login to view]
