Add some extra fields to a software already built.

Hi, good day. Im looking for someone who could help me on adding some fields on a system im developing. Those fields must be displayed when an user tries to search for a certain item (Screenshot #1) Also, the search field must be improved by letting your filter the items by some certain fields (screenshot #2). Basically all you need to do is add 2 extra fields. Those fields must have an option for add/ edit/ delete the info they register. Right now, already exists an option that does exactly what i need on the system. You can copy / paste that option and modify them to achieve this (screenshot #3). Im pretty sure you can achieve this on a few hours.
