Add phone verification on a page using Twilio + Some other small stuff. Read description

Background: I have simple 3 page website with a form on it and a dumb simple admin panel showing the data being inputted in the form (got it done for $60 so its really simple). I need to add some more functionality to this simple website. The list of functionality I want to add: 1) I need to add phone verification to a page using twilio inorder to submit a form [login to view URL] 2) I need to move a 3 page website and the DB to another hosting. Both hosting accounts have cpanel so this is super simple to do. 3) I need to make geo IP script work. I ripped a page and I cant get the JS for geoIP to work. You need to trouble shoot the JS and make it work like the other page. I'll give you the link to the page I ripped. 4) I need to send automated thankyou email after submitting form using mandrill https://mandrillapp.com/api/docs/messages.html#method=send 5) In admin panel when the shipping info is added in shipping info field I need to fire another automated with that info. 6) I need to add another "comments" filed in the DB and the admin panel so that we can use it for writing comments internally.
