add information to a mysql database without leaving the page

I have a website, that displays a number of job vacancies from a database, what i want to do is include a form in each of these search results which once the button in the form is clicked by a user it inserts some php variables in to a database. At the moment i have a brief working example with a form appearing in each search results. Once the button in the form on the individual search results is clicked it inserts the php variables in to the mysql database The problem is that the form doesn't reset or clear the information and it continually adds the same information in to the database time after time, even if another search result is clicked. What i am after is somebody who can fix the problem, i have an idea as to why this is an issue, however my experience with javascript is limited and i really don't want to spend weeks fixing it. I have a number of projects that need to be completed to help me reach a couple of deadlines and so it would be good to find a good all round developer. Thanks
