Create Search Form with Photoshop & CSS for Wordpress

This will be a simple task for someone that has these skills and should not take much time. I need someone to look at the attached example and create a similar form that I can paste the form code into and HTML page that will post and display the IFRAME results on a results page. What this entails is creating a photoshop form for the image part and then adding some code to the CSS file to dress it up. I need to have this form with career field, zipcode and or State search on the single form. The person will pick career field and either zipcode or state to search by. I anticipate that this will take about 1 hour.
Do not bid if you cannot handle this task. Hopefully I can get someone to start on this task right away. Hopefully someone can complete this task for about $30
