correct form to email on 3 websits - its the same script

3 sites using form to email, but its not working. The site is hosted on eclipse, and is can use either ASP or ColdFusion - I personally would prefer this to be done in ColdFusion.
It needs doing asap - I will select a winning bid in 5 hours max, which is 11 am GMT.
Error message shown in the further details, and I will provide FTP details to the winning bid. The zip contains the 3 contact forms as well.

Form to email is not working, the same error message (but with different page URLs of course) shows for each form to email.

Form needs to start working. You can change the form to test to run off your own email address, but then revert them to the clients email address please.
