convert PDF documents into editable versions for customers to use

This is a very simple project. We need someone to convert two standard PDF documents - 1 is 1 page; 2nd is 10 pages into editable PDF forms that users can save and print. The second one needs to have some basic smarts - based on a field on page one that triggers how many 'members' there are and therefore how many times to repeat 'new member' pages' I am attaching the two documents so you can review them.
