Adding Form Capability to 15 PDF files

We're redesigning some forms for an insurance company. The last step is to make them editable with Adobe Acrobat Reader. This would be your job, if you bid here. The forms are all just ONE page and have about 30 items each. (Not counting the top-part which always stays the same and can be reused.) At the moment I have 13 forms. Maybe I get 1 or 2 more. So the bid is for between 13 and 15 forms. Here is an example: [login to view URL] (This is about average complexity. Some are simpler, some more complicated) The task is to simply add those form fields in Adobe Acrobat and send the pdfs back to me.
