Asterisk+Kamailio+Realtime mySQL Integration

I will provide a fresh VM. I need the following done:
1- Install Asterisk v10
2- Integrate it with Kamailio
3- Install Realtime mySQL module
4- populate database with the sql backup attached
5- link the database + asterisk so we can register trunk, extensions and make/receive calls
End result is to have a production system of Asterisk, load balanced with Kamailio, as we have 1200 concurrent calls.
We must be able to populate the realtime database which we can register the softphone and make outbound calls as well as receive. With all other asterisk functions working such as conference, ivr, transfer in and out etc. Fully functional PBX.
We will be building our own GUI via PHP by populating the database. Being realtime, it must be actioned immediately.
All steps must be documented and a manual supplied upon completion on what was done to complete the project. This is required for our troubleshooting.
Please advise if you can build such solution and/or already have one.
