Create an HTML5 cross-platform app

This project has 3 parts: 1. Create an app 2. Setup a local environment for me to manage this app and make minor edits and recreate the app. 3. Basic training to create the apt files etc. I am looking at a simple app with the following specs: 1. Create a cross-platform app using HTML5 and phone gap (or any other suited tool) 2. The app must work perfectly across different screen sizes in Portrait as well as Landscape mode 3. There must be web interface to view/upload/edit the contents 4. Elegant UI/UX based on a few sample apps which are appealing. We can look at a few options and take inspiration from them instead of creating the design from scratch. 5. Menu options: A) Home B) Projects - Plan - Gallery - Amenities - Details - Map C) Contact (This must have a map and a contact form which must send an email to my id and an acknowledgement to the senders email id ) D) Upload form - The first field will be to select whether the user is new or already exists - If its a new user then create a profile of that user with few fields and a photo - If the user already exists then search for him and select the user - 20 or so fields to take down details of people interested at the venue itself - An option to click a snap of the person or upload one from the gallery - Is it possible to have an option for the user to sign on the screen and we upload it along with the form? This is pretty much most of the app. In addition to this there might be a few more links. We'd like to see these links as tiles on the pages. PS: Please bid only if you have already worked on HTML5 apps and are fully equipped to take this up. This is a small project but for us the portfolio of the developer matters.
