Design of Icons, Splash Screen for Software Application

We are looking graphics designer to redesign our clinical practise management software. You are required to design and deliver : 1. Product Logo 2. 1 Product icon (XP Type, 16X16 & 32 X 32) 3. 15 software module icons (XP Type, 16X16 & 32 X 32) 4. Suitable start-up splash screen 5. Suitable background image The bidders are required to submit examples of works done previous. All the outputs from the design will be used in a visual basic application. We will supply more details in private correspondence. Upon selection of the bidder, we will provide the following : 1. Software setup files (for you to understand the software and our requirements) 2. Attached is the software brochure (general description of the software product) Module Icons : 1. Patient List 2. Corporate Client 3. Doctor Details 4. Clinic Information 5. Item Master (Drug Inventory) 6. Diagnosis Details 7. Treatment Details 8. System Update 9. Clinicplus - Online Transactions : 1. Patient Queue 2. Visit Entry 3. Claims Invoice 4. Payment Entry 5. Statement of Accounts 6. Goods Received Note ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
