change my search bar design / move my script into a new template, just 2 pages to edit

I need you to change my search bar design my script uses two main files i need the script from these two main files moving into the index page of a new html template my script [login to view URL] you will edit the search bar feature on that page to look like the image below [login to view URL] this page you will need to load on the same template as [login to view URL] IE in the same place replacing the search feature but in [login to view URL] file move the php code from [login to view URL] and [login to view URL] into the new template [login to view URL] as seen in the image below I have tried to move the php script to the new template but as i dont understand about the java links etc i cant get it to work but im getting it where i want it I need this straight away
