Need a wordpress form as defined below:
1. row 1: Text field...Full Name (required)
2. row 2: Text field...Email Id (required)
3. row 3: Text field...Mobile No. (required)
4. row 4: Dropdown Menu...option 1, 2, 3 etc.
5. row 5: Dynamic Text field
5.1 Multiple words can be added by typing and hitting enter after typing each and can be removed individually).
5.2 Based on selection made in row 4, the placeholder text of row 5 will change to as per pre-configured.
6. row 6: Submit button..On form Submit, the details should be emailed to the admin and a pre-configured auto email should be sent to the user submitting the form.
