We require a range of eForms to be built and integrated into our website. The forms will have conditional logic statements depending on the user input. There are about 6 forms that need to be built. Ideally, the forms will flow into one user experience (e.g. Form 1 (click next), Form 2 (click next) and so on.
When a user is on the home page of our website, there will be a button that says "Start Completing Your Forms". When the user clicks on this button, the forms will load as a layer over the top of the home page (the home page is faded out) with the forms in front in a dialog box. The user will then complete each step 1 to 6 before being able to preview the entire entry before clicking submit.
On submission, an email will be sent to a generic email address that advising that the forms have been completed by the user.
The recipient of the email can then login to a secure section (we need a user name and password section created to allow for this) of the website and download the forms that have been completed by the user in a PDF format. They will also be able to view the forms completed in an online version. This will be presented in a table view listing all the entries submitted. The recipient should be able to search for a record using a search function. The record list should display in a table format the following:
First Name
Last Name
Date form submitted
Time form submitted
There will then be a view button on the right that allows the recipient to click on the submission to view full details.
This will require an SQL database that we can have created through our FTP site.
We will give you access to our site so you can see how the forms and the design can be integrated using existing design assets and elements. We will require the forms and site to be responsive allowing for form users to complete forms on any device.
