Build a Form Website

I am creating a website using the service "[login to view URL]". I have a form in my website that I would like to link to a pdf document that the user signs. I need someone to help me program the website to do the following: -When the user fills out the boxes and then clicks "submit", the pdf will be populated with the fields that the user then signs I need someone that has worked with Rightsignature or Docusign to do this by integrating it into the website or using an API or however. I would like to keep designing it in Wix so if we can have this tie to rightsignature or Docusign that would be great.
