Create 2 interactive PDFs with embedded video and form fields

Hi, I'm looking for a skilled individual or company with a high feedback score (4.7 and above), 80% completion rate, and the proper experience (please provide examples) to take on the following project: - Design 2 interactive PDFs in Adobe Acrobat. Each will have an embedded video (35-40 seconds long, hosted by me) on the front page, will have a clickable table of contents, 5 content sections (I will add all the content), and a form field (6 fields) on the bottom that I'd like to send the filled out info to my email. I'm hoping to have this done today if possible, and would like the finished file so I can fill in the content easily.
