Adobe Live Cycle Design Project

I have a form that I need formatted to match my business identity, color scheme, feel, etc. The form will be sent to clients and prospective clients and needs to look as absolutely professional as possible. The forms have been constructed regarding the fields and content, however, the color, layout, etc is what I need help with. Basically, I need the form to look very good and professional. You MUST have Adobe Live Cycle Design to modify the form. I have included some material from my business to get an idea of how my marketing material looks and the professionalism that I need to convey when sending out any form. The pdf is for getting an idea of my firm. The .tds file is a VERSION, not the complete version, of the form that I will need modified regarding layout, color, flow, etc. Please let me know if you have any questions.
