Create Microsoft Excel sheet for my 36,000 subscribers list

I need someone to take a Microsoft Word document that has 36,000 Subscriber ID, Name, Email, Has Sent, Mail Format, Sign Up Date, and I want them put in an excel file. Each row should be a different contact. Each column across will be the following six categories listed above. I simply want this converted from Microsoft word document to an excel file clean and easily readable. So I can export this excel file to my Auto responder service.
