Create Electronic Form in Adobe Acrobat

I need a coder to create an electronic form in Adobe Acrobat that I can email (a PDF document) to people and they can fill in the answers in the appropriate fields, and either save their data in this form & email it back to me (if that is possible), or print it out (looking neat & professional) and post the paper form back to me.
It must work on versions of Adobe Acrobat Reader as far back at 3 or 4 (although if this is impossible, then whatever the earliest version it will work on is fine) - please advise me of what version of reader it can be used in when submitting your bid.
While the form doesn't have to look like it was designed by an award winning graphic artist, I do want it to look business-professional.
## Deliverables
The form is meant to capture information about businesses applying to join a service I offer.
This means I will need all the obvious fields, such as name, address, post code, email address, phone number, etc.
Please note that this is for the Australian market, so the document MUST be A4 sized portrait-formatted. I will provide specifications for field names & sizes to the coder winning the bid.
Next, the form requires several fields where the applicant can add free form text information describing their business & its offerings. Typically, this field will require approx 500 characters of space for use.
Once the applicant has filled out the form, I'd ideally like it to be able to save the data in the document, and then email it back to me for printing (or copy & pasting). I'm not actually sure which option Adobe provides, so you will need to advise me of this with your bid. That way, I know what I am getting.
Keep in mind that any person filling out this form is ONLY going to have Adobe Acrobat Reader (or Adobe Reader as it is called now) on their PC, so it must work with the lowest common denominator.
The form should look business-professional (I will supply business name & address details to be added to it), but at this stage there won't be a logo since I have not yet had one created.
