Application of an eBook Template on an already existing electronic manuscript (Autobiography of Saint Therese)

Hi. I would like an attractive looking e-book template applied to the autobiography of St. Therese of Lisieux. The text of the book can be found here: [login to view URL] I would like all references to project gutenberg removed per the project gutenberg license. I would like a colorful or otherwise pleasant e-book theme used. I would like a .pdf version available for the final product. I would like to be able to look over and make aesthetic changes to the book prior to the final copy pdf being rendered. I would like a Table of Contents that links to each section and each section to have back links to the t.o.c. I would like the foot note numbers to link to the footnotes section and the footnotes themselves to link back to the appropriate spot of the book.
