Convert three 4-page PDF forms to XHTML

We have three PDF forms, each four pages long. These forms have been compiled into a single, twelve page document. We require each form to be converted into a single XHTML page (resulting in three pages - one for each form). We do not require any programming, and the resulting page will not be an XHTML form, but rather will contain placeholder tokens in the form of {token} where HTML input tags would normally go. The PDF sections are split into logical tables, which should remain. All design elements should be replicated from the PDF as closely as possible (layout, colours, tables).
