Generate Reports From Text Files

I am looking for a small converter script that will allow me to load in a text file and then automatically generate a report from the file text.
It really only needs three buttons "Load" which will allow me to browse the file, "Clear" if I load the wrong file and a "Generate" which will run whatever it is that converts the text into something like a report.
The report itself can load directly into Word or Publisher formats. Or possibly just print directly from the text into a completed report. You tell me which will be the easiest method. Each line should be able to fit on a single A4 line as in the pub file.
I have attached two text files as I can generate the text file in either way. I have also attached a publisher file for a certificate that I entered the figures in manually. A small software application that will ease this burden for larger amounts as in the example txt's will help me greatly.

I have been told that Crystal Reports may be of some use with doing this but I have never used it???
