Need a web application - that fully responsive to all mobile devices

We have a client that is looking to create a way for their customers to apply for service witout ever having to see the. The client is a state funded program that provides energy assistance to low income families and they also do weatherization assistance, the two programs have seperate times of the year the programs run and you can only apply for one program. Core funtionality: 1, Application will need to have a backend database that can determine if a new sign up is new and if not send a message to user that they already applied to this program. 2. We do not want users to have a password the system should recogonize them based on last, name, and two other identifiers we will provide to developer upon hire 3. once a user is determined to be new they should then proceed to the application. 4. Application will collect user information as needed to state application forms 5. need back end client login with 3 permission levels a. processor - client users that can collect and send applications to proper agency, view reports, send reminder emails to applicants on missing or needed paperwork. b. client admin - ability to create processor level accts, change processor passwords, gernerate reports, send reminder emails to applicants on missing or needed paperwork. c. Super user - developer account to administer all aspects of the application 6. Reporting system 7. preset and custom message mailing [login to view URL] want to brand the application for this client but we also the ability to duplicate the application front for re-purposing to other clients, so the application should be built with intention that this is for multiple customers and will be re-branded over and over, so the application should work independent of the form it has to populate. The intake application should be a stand alone feature that collects data, the data should be populated to the forms based on the data sets collected and how we name the form sets so that any form we need populated will work. We will provide a cloud server and dev domain for development purposes. Developer will be required to perform all work from the provided server. We need two quotes for this project: 1. quote project as a responsive web application, and include rebranding for new customers and on going support 2. SAME AS ABOVE EXCEPT - quote as a mobile application iOS and Andriod, with publishing to both app stores I am going to be posting an updated design spec document by Tues the 12th.
