I need to be able to allow site visitors to enter details to request an appointment from front end. Once they have entered all information required and hit submit it needs to send notification email with all information they provided to email set by administrator from contacts list.. Front end needs to display calendar that they can select date from and best time to call Admin section needs to be able to let me modify what fields to display and what email address to send to from Joomla contacts list. I need this as a module that I can upload and assign position to. All work to be done on your own developer server. Sample image attached as to what I want.

In component you will see all the submission and in module the appointment form in. All things will be configurable/editable from backend.
