javascript form maker

REQUEST: Produce a single HTML page that can be saved and run offline. It should be written only javascript, css, and html. It should be able to be saved and should work offline. A mockup can be viewed at [login to view URL] The core javascript code to be modified is attached: [login to view URL] TOP: A large area of plain unformatted text. This is the “soapnote markup” described at [login to view URL] The text in the textarea can come from... typing it in, or opening a text (*.txt) file, or selecting a Google Doc The text may be saved... as a text file, or as a Google Doc BOTTOM: A functional form which displays like the forms at www.soapnote.org. The form should work exactly like the forms at www.soapnote.org. However, there is one feature that does not need to work on this page - the prepopulated text feature (this is explained at [login to view URL]) Note: the forms at [login to view URL] produce output text from the form inputs. EXPLANATION OF MOCKUP ITEMS (from top to bottom): "help" links to [login to view URL] "markup" is a large textarea "open" prompts to open a text file or Google Doc "save" prompts to save to text file or Google Docs "make form" makes an HTML form from the markup text and puts it into the "form" area "form" is the working form "export" prompts to save the form as an html file that works offline
