FPDF expert required

Hi, I need someone who has worked before with FPDF to generate PDFs from HTML / PHP code. The output needs to be in several columns and no hardcoding is acceptable. Please ONLY apply if you have worked with FPDF before to save your and my time.

I need a fpdf like the pdf attached which can work with the dynamic data from database with header on first page and footer on all pages and then I will import the data from database accordingly.
