Fix some issues/add functionality to pages(s)

I have a form that is used to submit entries on a weekly basis. The form can be submitted once a week, or updated throughout the week with changes. There are some parts of the form that need to be fixed, and some added functionality I would like to incorporate. This is an American Football based football script, where you make selections each week. There are also "side pools" that users can be entered in, where they would also make a selection for. Requirements: 1.) "Vegas" pool - Currently you start with 100 points and you "wager" x amount of points each week. If you win, you get credited, if you lose, you are subtracted that amount. The current drop downs for team and wager amounts need to show "Eliminated" if their points = 0 2.) "High 5" - Select 5 different teams each week. All that is needed here is to do a validation of those 5 picks to make sure that the same team is not selected twice, if so, and error or just a warning message pops up. 3.) 3 separate pools that need same functionality. Each of these pools, you select 1 different team each week. If your team loses, you are eliminated. What I want to do is allow for 1 loss within the first 4 weeks and give an option to "Buy Back" in. This means, if the person loses in the first 4 weeks of the season, they would have a checkbox to "Buy Back". If they buy back in, they are allowed to make a selection again, until they lose again. After week 4, the option is no longer available. If the user uses the "Buy Back" option once , they cannot use it again. 4.) Fix a very small php DB info script (since upgrade to newer PHP, getting errors. Script is only about 10-20 lines long.
