Design HTML input form for a ready made theme

I need a HTML UX/UI Desginer to design a form for a ready made theme. The form is normal input form for "new Event" The theme can be downloaded from here [login to view URL] theme name: The conference The form should contain the following fields: Event title -> text input field photo -> photo upload field date -> datepicker field city --> text input field address --> text input field about --> textarea input field speaker name -> text input field speaker title -> dropdown list of titles speaker photo -> photo upload field about speaker -> textarea input field a small grid to list speakers added to the event The design of the form should be done professionally to like as part of the theme
