Database on our web host

Hi We want to create an SQL database on our web host. We would like from you the installation and setup of the database and a short explanation to us about the simple handling of the database. The database must be set up so that we can enter additional properties (entries) ourselves. Our web host is called: [login to view URL] The database menu is called phpMyAdmin. Details about us and the required database: We are a maintenance facility property management. Each of our properties has a form with tasks that must be completed within a year. We want the forms to be stored on our database. At the end of the year we have to show the customer (owner of the property) this form in a PDF that all work has been done. We imagine two variants, maybe you have a better variant: Version 1: We create 1 form for each property. Each forms can be opened on a separate link. On the form there are multi choice fields, the date and time when each of the tasks was done. It would be great as soon as our worker has clicked on the multi choice field of a task, the date and gets filled out automatically. It would also be great if our workers can not change any previous entries in the form, which were not performed that day. That way we avoid misadventures. This form can be printed as Excel or PDF at the end of the year for the customer. Variant 2: We create a complex database where all tasks, list of the properties etc. are created in seperate lists and you sync them together into forms. In anyway: It is important that our employees can reach via link on their mobiles to the forms and the forms are simple to fill out. We look forward to your offer!
