Create a web upload app for archiving task

My goal is to have a webform with multi file upload with MYSQL backend + search web form and retrieve documents - the requirements are: 1. Single [login to view URL] or [login to view URL] to adjust storage paths, hostnames, sql, email SMTP details and alerts email contact, webform field names and conditions - anything that can be human editable lives centrally (can be a admin config area webpage) 2. per file upload requires title, description, keywords added to SQL - eg users wants to upload 3 items. 3x items to SQL insert and 3x items to put on file path directory. 3. upon file submission uploaded attachments/items renamed to next prefix index number from SQL and random characters to remove user naming schema and issues but keep extension type suffix. (ie [login to view URL] becomes [login to view URL] to storage path) now omitted 3.1. I would like OCR scanning of PDF items and if possible opendoc standards / MS xls xlsx doc docx items. I realise this increases CPU overhead and SQL table size but that a nice goal. 4. search will be a search by title, description, keywords. Combination AND and OR search function required. 4.1. search ocr results if OCR implementation possible. 5 sql must be well managed, if operating separate tables to improve search performance, must also be multi-user functional ie upload and search must function while another users is doing similar tasks (no table locks that affect other tasks) 5.1. sql ensure index/tracking maintained across all SQL tables for recovery purposes. (worst case scenario) now omitted 5.2 sql ocr data (if ocr available) if sql table data sizes get large making useful searches slower over time, keep separate search task only for ocr content so normal search speeds using title, description, keywords search is separate search tasks from ocr search - you can fade/disable title, description, keywords search fields when entering content into ocr field search field on web-form. 6. overview with be site with front page is search fields, and a upload page/upload tab item 6.1 web form must be ready for mobile and desktop browsers 6.2 web form design, simple, as per item 1, headers, fields, must enter criterion etc editable in config area. obviously some design samples can be sent through but a form is a form. I will sort the mysql, file storage and hosting. does not have to be 100% custom, if you already have a working building block, this would be better supported long term, use it.
