I have ten different forms which I wanted to be converted into soft copies so that data can be extracted from my PHP developed Electronic Health Records(EHR) system. What I want is for the data to be retrieved from and entered into the EHR system using the layout on each of the forms attached. The forms will have headers with logos and contact details which I will give to the winning freelancer. I will need to retrieve information from the system and the information will be printed out and come out exactly as the layout on the each relevant of the 10 forms. The printout should be laid out exactly as the attached forms.
When you reply, I want the following information;
1. Time you may need to complete this job
2. Your charges for the complete job or your hourly rate plus the number of hours you feel you need to complete the job
I will give the winning freelancer access to the EHR data. This job needs to be done as soon as possible but please don't apply if you can't do this job.
