I need a form built using table from mysql database. User enters values from a web page and saves. Saved data is written to mysql with date/time for editing again later or exporting as a CSV. Table is total of 3 columns, 2 are read-only from the database, the other is a column that is editable text. I've attached a mockup of what the webforms should potentially look like as well as the resulting CSV. Please let me know if you require clarification. We may add more features w/ additional milestone

This sounds like an easy project. I could have this done in under 2 hours, including full jQuery saving of the fields that have only changed, for optimal client/server data transfer. If you are going to have a large number of database entries to display, I'd recommend pagination or lazy loading.
