Add image upload to laravel form

I need someone to add the function to allow user to fill out a form and add up to 5 images which the info then gets stored in a database.
I need the image path stored in the database too so I can display them along with all other information.
I've created the form, just need help with the file input.
Must be able to work with laravel 5.
Need this completing within 24 hours
