App for Ipad

Would like an app to allow user to enter records for the following personal data This part should take up left 50% of the screen Person: Record X of XXXXXXX (just a count of the total records stored) First Name: Textbox Middle Name : Textbox (optional) Last Name: Text box Age: (scroll) 1 thru 99 Height: (scroll) 2' ... 2' 1'...2' 2" etc... 2 foot to 9 foot Weight: 65 lbs to 499 lbs (scroll not textbox) Should have left and right buttons to move thru records This should be most 50% of the right hand of the screen) Only one large photo should take up right side of (50%) of screen (landscape) Add Photo 1 (Face) Required*** Add Photo 2 (Whole Person - Body Shot) optional ..default No Pic Available Add Photo 3 (Left Side Profile Body Picture) optional ..default No Pic Available Add Photo 4 (Right Side Profile Body Picture) optional ...default No Pic Available should have 4 Add PHOTO buttons under ONE large picture allowing user to click on Add Photo 1, Add Photo 2 Add Photo 3 or Add Photo 4 should have 4 SHOW PHOTO buttons under ONE large picture allowing user to click on Show Photo 1, Show Photo 2 Show Photo 3 or Show Photo 4
