Drupal v6 - User permissions are not staying applied

The kind of issue you are talking about is not very common to us. I can not remember if we faced such an issue earlier. Therefore you have rightly identified we must have to go through the debugging process to find the cause and solve it. hook_perm() is the hook those we use in the admin for programmatically add permission. If you are using ACL module for restricting users access , there might have some conflict with the permission enforced by your module and the the ACL module causing permission to become un-applied. There might have some other issues like adding permission within t() etc. We use different tools for debugging like contributed module Devel, the Drupal for Firebug contrib module and Firefox extension and Xdebug. We will need the access to the drupal website to solve this issue of unapplied of permission. Please also send the occurrences so we can replicate the issue.
