I have an example .publisher document attached which is an invoice that gets printed off for clients;
I require the following;
1. When printed off it must stay in the same format [some modifications can be made if needed]
2. Each question should have a text box for the answers to be added that does not alter the format and makes easy to type out.
3. Date must change on the date a new form is opened for completion.
4. The final costs would be calculated with the fixed fee [fee must be editable]
5. The final costs would be calculated with the daily rates. [daily rate must be editable]
6. A data sheet on excel is available with the name of all the banks. Must be able to pick a bank from a drop down or auto populates as you type.
7. Each bank has branches, once the bank has been selected the branches should be available from a drop down that matches the bank chosen.
8. Able to save in a read-only format [for security] and a version that allows modification.
9. Provide Unique ID for sender, that when input later will populate name, address, postcode, telephone, identification.
10. Provide Unique ID for receiver, that when input later will populate name, tel, collection bank, branch, identification.
