Basic Asset Database with Check in/out form function (SQL/Access)

Please only bid if the expected completion is before 7th July. I require a basic SQL database with a Access (or similar) front end for a simple check in check out system for assets(laptops). There are only 5 or 6 columns on 1 table. The scenario is that using a form, an asset is checked out/issued by entering in its MAC address (unique) and some personal details about the user it is being issued to. The Asset it then marked as 'Checked Out' and a timestamp applied. When the asset is returned, it is simply marked as In Stock. Requirement is SQL Database, 1 form for Issuing assets. 1 form for Returning assets and a report to see what assets have been issued and returned. See attachment for screenshot titled [login to view URL] for the fields required for issuing assets and [login to view URL] for the form to return the asset. Note the checkbox 'Reload Last' on lending page. This queries the previous Lend on click for the 'Collected by' 'Contact number' 'booth' and 'note' fields of the previous record. This is because multiple items/'mac addresses' can be issued to the same user and this saves retyping that information. Ideally the work would be completed by Saturday 7th July.
