Create Order Form Creator

Please take a look at this order form: [login to view URL] It was created with a simple order form creator. The page looks like this: Click Here to Create New Order Form Next Page: What information would you like to collect from the customer? (see order form above - these are all checkboxes) What items would you like to sell? It pulls the items from the API (using SOAP) What shipping would you like to add? It pulls the shipping code from the API (using SOAP) Would you like to put any links in the bottom for policies? Enter HTML code for bottom.. Would you like to customize the order form to make it pretty? So I need a new one created for a new order taking system that I'm using. If you know how to do this where the order page looks "cool" (not the black with simple forms like the example) then please bid on this.. I'm looking for a "order form creator" that makes it easy to create order forms on the fly and tie into any website. Thanks!
