Customize Openconf php portal , php mysql database

I need some customization on [login to view URL] I made some but I need help. I'm going to send my code and database to you. Modifications are easy; * Changing the context of submission page ([login to view URL] and [login to view URL]), must have relations with database, and must work with no problem. * Remove footer from all pages I attached the submission page that I want. PHP and MySQL knowledge needed. Changes should be in database and php code. All changes must told to me so I will apply to my own code. You must install the portal to your own development environment and provide me a link to view and test changes on site. In submission page the biggest problem is there is only one field to upload file, I need two upload parts and one must be doc or docx format other must be only pdf format. Details are attached.
