Create 'File Upload' Form in PHP

I need a form created for [login to view URL], a crowdfunding site to raise money for a nonprofit venture in Indonesia.

Participants can simply donate money (we have that taken care of), or they can submit a design and donate $25.

We now have two forms on the site. One, at [login to view URL], gathers information about people who want to enter the design competition. A second form, at [login to view URL] , allows them to upload a file.

The use of the two forms is a temporary measure. I want to replace them with one form.

The new form should accomplish three things:

1.) Capture the person’s information, as now on the [login to view URL] page.

2.) Allow the person to donate $25, and return to the form to continue (or this can be the last step in the process.)

3.) Allow the person to upload files to the website, restricted to files in JPEG, PNG, PDF and DWG format. File size can be fairly large – 10 megs, maybe larger.

Somehow we have to link the sender’s name to the filenames of the files they upload. One possibility would be for the form to combine the sender’s first and last names and the date, and append that to the filename supplied by the uploader.

It would be nice to provide a confirmation page showing the information the site visitor provided, and the success (or failure) of the file upload.

The site has PHP 5.2.17 and MySQL version 5.5.23-55.

There are several open source PHP file upload scripts that could be the starting point for this, such as the ones at [login to view URL], [login to view URL], and [login to view URL]

Because this is a non-profit operation run by volunteers, the budget for this project is very small. We would be happy to provide a credit line on the website.
