Collect info from web form & create PDF with data from form

I have a PDF form on my WordPress website that needs to be turned into an interactive form. The form needs to be able to collect a new member's information from a web based form, which displays the information as one form field per line down the page. The information collected needs to be stored in a database so it can be used later for statistical and other reports (the reports are not needed now, just the collection and storage of info in the database). Using the collected information, a PDF document needs to be created on the fly that the member can print, sign and send to us via snail mail. **The flow of the project** Member enters website > Clicks on join and is taken to a page with the form fields on it > The member fills in the form and submits the information > The information is written into a MYSQL database and is stored for future use > The website redirects them to a thank you page that says their membership application has been created and to click on a link to download, print, sign and send the form to us. > The form is created when they click the download link and opens in a new window ## Deliverables Please make sure you acknowledge that you have read and understand the job description before applying for this job. The form must be laid out as close as possible to the attached PDF document, with the following details collected. 1: Party Name (Rights Party of Australia to be displayed and can't be changed) 2: Applicant Surname (this is a mandatry field) 3: Applicant Christian Names (this is a mandatry field) 4: Residential Address (line 1) (this is a mandatry field) 5: Residential Address (line 2) (NOT a mandatry field) 6: Phone No (Home) (NOT a mandatry field) 7: Phone No (Work) (NOT a mandatry field) 8: Fax No (NOT a mandatry field) 9: Email (this is a mandatry field) 10: Birth Date (dd/mm/yyyy) (this is a mandatry field) 11: Date (this is a mandatry field, but should default to the current date) All of this information needs to be used to create a form that has been filled in with the members details, ready for printing. It needs to be created as a PDF document and look as close as possible to the attached document.
