Combine two php forms that interact with database, into a form that just sends an email

I have two PHP forms that interact with a database driven admin section. The first PHP form is a registration form which collects contact information. The second form is a quote request form. I want to create a simple quote form based on these two php forms, that will email the contents to me. ## Deliverables Please note: validation for all fields is requested, so that if for example, a badly formed email address is entered, it will show an error and set focus on the email address. The existing forms have this type of error checking (however they connect to a database, rather than email the results). there will be up to 3 email address to send the form to. I'm attaching screenshots of the 2 existing forms (and a cut and pasting job done for the final new form). These screen shots should give you a good idea of how things work. The country drop down menu brings up a state drop down menu for American states if USA is selected. The required fields will change a little from the "[login to view URL]" file and I'll have the final required fields available very soon. There is an includes file I believe for the header and I'm sure there is one for the footer which will bring in the logo, navigation menu, etc... Please bid for both time cost. Time is of the essence here, and I would like this completed by this Saturday EST if not sooner.
