database to pdf file

We are looking for a coder who can pull records from a database (MySQL) and create an Adobe pdf file from it. There is an open source php software to achieve text or html to a pdf file - and is used on many websites. We want the input to be content from a database. For example, columns 1 to 5, for 100 rows, to saved as a pdf. The volume of content are not the same, the description column could be one sentence or a few paragraphs. Ideally, the layout of the pdf will be two columns on a page, consist of a header for the first database column for each row and smaller fields underneath it. The format must be [column 1] (the title - probably h3) [column 2] * [column 3] * [column 4] (3 short database fields spaced with a "•") [column 5] (the description) You will need to create a dummy database of around 40 or so rows to test, we can provide a dummy database if you prefer. You must be able to provide a working prototype on your own hosting before release of funds.
