Convert 3 Rental PDFs to Web Based Forms

We have 3 rental agreement forms we would like to have converted to Web Based forms. Basic Tenant Information will be saved and used for all three forms. The other fields will relate to the type of form (Holding Deposit, Rental Agreement, Pet Deposit) and have fields unique to each of them. Some fields may need the option to add more than one answer for it - for example, if there are 3 people renting, then the Tenant Name field would need a "add tenant" button which would create a new Tenant Name field. Forms should handle single tenants or multiple tenants. Form information will be saved in MySQL tables using PHP (we will adapt some of this later, so PHP/MySQL must be used for Server Side language). Once the form information is submitted, a separate PDF for each original (paper) form should be created. The tenant or tenants will then sign digitally using a Signature Pad (html5 canvas or other solution) or by using a saved signature image file. When the signature is saved, the PDF document should have the signature placed on the Signature Line of the document. Both the form field information and the saved signature image file should be saved and stored on the server, along with the generated PDF file of the signed document.
