Need someone to help me create a simple uploader form that will store data into my db and send me an alert email with form input info and download link for uploaded files and a page with pw to login to view data and let me download uploaded files. sample form: [login to view URL] sample data page: [login to view URL] (missing some data - customer name , company name, phone) my budget for this project is between 50 - 75 USD need this to be done asap so the bid with lowest price and fastest turnout time will get my project.

The uploader form and the subsequent processing as described can be developed. Alert email option with download link for the uploaded files and the password protected page to view and downloaded the uploaded files can be developed.
