Create Kickstarter Pledge calculator - Quick turnaround

We want to create a simple calculator to help our backers determine their exact pledge level based on the options the choose. We are looking for a file we can add to our website (i.e. [login to view URL]) and where backers will be able to visit and use the calculator. We would like to have a calculator that is very simple and present all the options to the user to determine the final cost. At the end a summary of the options is displayed with the total.
