Attached is a reduced draft of a database I am working on for a school project. This database is basically supposed to keep track of all trouble calls related to system administration. I am looking for a way to keep track of personal information of sys admin, user requesting help and the trouble call itself. I want there to be a form related to each database and I also want a query to be able to use to look up any type of information that I could possibly want whether it be any past tickets a user has submitted or similiar problems with a particular computer. Relationships set up from the Remedy database to contacts and Employees would be great as well. Any ideas you may have regarding this is greatly appreciated as well. I have the basic entities already in place and others may be added as long as it retains to the project description. I Also require test procedures so that anyone would be able to understand how to use this database. Anticipate that entry level system administrators will be the ones using this in order to keep track of thier trouble calls for documentation purposes. I would also like it to where only certain users that input the information be able to change. I don't want another sys admin to be able to change someone elses input. There will also be the supervisor who should be able to access any of the info.

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
