- We would like you to construct an adobe ‘PDF’ Fillable form from the file we have sent you - Each page on the file we have sent you has a number in the top left hand corner. Where there is the same number on a page, we would like the information on these pages to be condensed into one page if possible. - Each question should have a free form text field for its answer, unless we have specified a ‘drop box’ or a ‘YES/NO’ checkbox option - We would like to be able to tab between sections in the form - We are leaving the design of the form up to you however we would like something easy to follow, compact and user friendly. - We are currently finalizing a new logo and would like you to allow room for this and company information on the top right hand corner of the front page (email:will be supplied Phone:)
