Add a contribution on OS Commerce Website

Need to add the Separate Pricing Per Customer Contribution ([url removed, login to view]) to our website www.southernbulbs.com.
We would also like it configured to have an e-mail sent to us when a wholesale account is requested so that we know to log in and approve.
Also, when creating an order for a customer through the back end administrative side of the website, we want to make sure that this wholesale pricing is able to be applied to these customers (see attached photo).
