Appointment Management system through a web portal

I need the creation of a simple but very simple for demostration and education purposes of a portal for making appointments thru a web portal where customers can make a book of pets. Customer can create username and password, login and then fill up the pet information and the patient medical record. Just a few pages where I can customize later as long as it has the funcionality of insert, delete, update of SQL with the appropiate connection. As I mentioned, The project will present a web platform for the customer to access through your username and password to a profile in which you can make appointments, view your medical history, reschedule appointments and cancel appointments for your pet. Some of the requirements are: Provide the customer with a user name, password and the ability to recover in case of loss. As I mentioned, this is for an education purposes environment so that is why the price, Im not asking for a complete and entire system. Just a few pages as long as I can demonstrate the use of INSERT, DELETE AND those SQL staments by using JSP and JAVA and glassfish or tomcat and SQL server. he client can register an unlimited number of pets (no limit).  Customer may make scheduling the appointment for your pet from the web platform.  The veterinary services offered will be present at the time of the conclusion of the appointment, if the service request is hosting it will provide an electronic calendar in which placed the start date and end date for the service.  The operator count on a friendly platform to perform the maintenance that apply, as the entrance and exit of the pet as well as logging in information related to a query has been performed.
